US West requested a temporary extension of its waiver of the single bill requirement for only those cases in which its meet point billing contracts with other participating non-Tier I local exchange carriers have been cancelled, and the parties have been unable to reach an agreement on new terms and conditions. US West also filed on November 1, 1994, a petition for rulemaking (RM No. 8540) to eliminate the single bill requirement. On October 3, 1995, US West filed an amended petition for waiver of the single bill requirement to replace its earlier petition. In addition to its amended petition for waiver, US West concurrently filed an amended petition for rulemaking to eliminate the single bill requirement. This amended petition for rulemaking is intended to replace its earlier petition for rulemaking. On October 5, 1995, US West filed an erratum to its amended petition for rulemaking. Interested parties may file comments on US West's amended petition for waiver of the single billing requirement and comments on the amended petition for rulemaking no later than October 26, 1995. Replies must be filed by November 2, 1995. An original and four copies of all comments and replies must be filed in accordance with Section 1.51(c) of the Commission's Rules, 47 C.F.R.  1.51(c). In addition, one copy of each pleading must be filed with International Transcription Services (ITS), the Commission's duplicating contractor, at its office at 2100 M Street, N.W., Suite 140, Washington, D.C. 20037 and one copy with the Chief, Tariff Division, Room 518, 1919 M Street, N.W., Washington D.C. 20554.
