This waiver, if granted, would increase Midvale's annual USF support limitation. On July 23, 1993, Midvale and U S WEST filed a joint petition for waiver of the definition of "Study Area" contained in the Part 36 Appendix-Glossary of the Commission's rules. The requested waiver was granted on February 17, 1995, to allow U S WEST to remove one telephone exchange from its Arizona study area. The requested waiver also allowed Midvale to add the purchased exchange to its existing Arizona study area. The waiver was granted subject to the condition that Midvale's annual USF support shall not exceed its forecast amount of $118,369. Midvale is requesting that its annual USF support limit be increased from $118,369 to $252,621, to reflect costs it incurred from July 1, 1995 through June 30, 1996. Midvale also requests that the National Exchange Carrier Association, Inc. be directed to begin, effective January 1, 1997, to distribute USF support in accordance with the increased USF support limitation. Interested parties may file comments on the petition on or before February 19, 1997, and reply comments on or before March 3, 1997, with the Secretary, Federal Communications Commission, 1919 M Street, N.W., Room 222, Washington, D.C. 20554. Comments and reply comments should reference AAD 97-23.
