UNIVERSAL SERVICE WORKSHEET, FCC FORM 457

On July 18, 1997, the Commission released Changes to the Board of
Directors of the National Exchange Carrier Association, Inc. and Federal-State
Joint Board on Universal Service,
Report and Order and Second Order on Reconsideration, CC Docket Nos. 97-21 and 96-45, FCC 97-253. In that Order,
the Commission released a draft copy of the Universal Service Worksheet, FCC
Form 457. The Worksheet requires contributors to universal service support
mechanisms to submit information regarding their end-user telecommunications
revenues. Official release of the Worksheet was conditioned upon receipt of
approval of the form by the Office of Management and Budget (OMB). OMB
approved FCC Form 457 on July 31, 1997. The official
Universal Service Worksheet can now be btained from the Commission's website (www.fcc.gov).
All entities that are required to contribute to universal service support
mechanisms must complete the Worksheet by September 1, 1997.
Completed Worksheets must be mailed to: USAC -- Universal Service
Administration, 100 South Jefferson Rd., Whippany, NJ 07981.
