Washington, D.C. 20554 November 9, 1998 MASS MEDIA BUREAU RELEASES NEW FCC FORM 302-DTV A new FCC Form 302-DTV (Application for Digital Television (DTV) Broadcast Station License) has been printed and is now available for use by DTV permittees. The August,1998, edition of this form has been approved by the Office of Management and Budget. Since some applicants may have nearly completed the old license application form, the Commission will continue to accept DTV license applications prepared on FCC Form 302- TV until December 31, 1998. However, applicants are encouraged to use the new form as it is a much simpler form to complete. A separate "Remittance Advice" form (FCC Form 159 dated July 1997) must also be submitted with any FCC 302-DTV requiring the filing of a fee. This new form may be obtained by writing to the Commission's Form Distribution Center, 9300 East Hampton Drive, Capital Heights, Maryland 20743, or by calling telephone number 1 (800) 418-3676 and leaving a request on the answering machine provided for this purpose. The new FCC 302-DTV is also available by using the FCC Fax-On-Demand system or through the Internet. Copies may be ordered via facsimile 24 hours a day by calling telephone number (202) 418-0177 from the handset of any fax machine. The document retrieval number is 003020. Follow the system voice prompts and enter the document retrieval number when requested. Due to the limited number of telephone lines into the forms Fax-On-Demand system, callers may wish to call during non-business hours.
