                                           NEW FCC FORMS 314 & 315 AVAILABLE FOR USE


A new FCC Form 314 (Application for Consent to Assignment of Broadcast Station Construction 
Permit or License) and FCC Form 315 (Application for Consent to Transfer Control of Entity 
Holding Broadcast Station Constuction Permit or License) are available for use.  These revised 
forms incorporate the new commercial broadcast ownership and attribution rules which become 
effective November 16, 1999.  These forms have been approved for use by the Office of 
Management and Budget.  

The revised FCC Forms 314 and 315 are effective immediately and should be used by all 
commercial broadcast station permittees and licensees.  All previous editions of FCC Forms 
314/315 are obsolete and should not be used.

The revised FCC Forms 314 and 315 are available via the Internet by accessing the FCC Web 
site at http://www.fcc.gov and clicking on the Forms link.  The new FCC Forms 314 and 315 
are also available by using the FCC Fax-On-Demand system.  Copies may be ordered via 
facsimile 24 hours a day by calling telephone number (202) 418-0177 from the handset of any fax 
machine.  The document retrieval number for the FCC 314 is 000314 (form and instructions) and 
003141 (worksheets).  The document retrieval number for the FCC 315 is 000315 (form and 
instructions) and 003151 (worksheets). Follow the system voice prompts and enter the document 
retrieval number when requested.  Due to the limited number of telephone lines into the forms 
Fax-On-Demand system, callers may wish to call during non-business hours. 

Printed copies of the revised FCC Form 314 and 315 (September, 1999 edition) may be obtained 
either by writing to the FCC's Form Distribution Center, 9300 E. Hampton Drive, Capital 
Heights, Maryland 20743, or by calling Telephone Number (202) 418-FORM or 1-800-418-
FORM and leaving your request on the answering machine provided for this purpose.
