                DISTRIBUTION CENTER ANNOUNCES A TOLL-FREE
                   TELEPHONE NUMBER TO ORDER FCC FORMS


To improve the quality of our customer service the FCC's Forms
Distribution Center now has a toll-free number to order FCC
Forms; the number is 1-800-418-FORM (1-800-418-3676).  Customers
outside the Washington, DC metropolitan area may use this number
to order FCC forms.  Individuals in the Washington, DC area
should continue to telephone 202-418-3676.  Commission employees
should not use this number to place orders.  Special procedures
are being implemented to assist FCC field offices and other
internal staff orders.

This toll-free number activates an automated response system
which allows callers to order FCC forms, hear a listing of most
commonly requested FCC Forms, and provides telephone numbers for
additional Commission services.

Callers requesting forms should be sure to provide a complete
address and telephone number, including area code and ZIP code. 
This will facilitate accurate processing of orders.  Please allow
2 weeks for order delivery.

In addition to providing these services, callers may obtain an
index of FCC forms available via FAX.  By choosing option one
during the toll-free automated response, you can have this index
sent to a FAX machine.  You can order the FAX copies of the forms
listed on the index by contacting 202-418-0177 from the handset
of your FAX machine.  YOU MUST BE CALLING FROM A FAX MACHINE TO
RETRIEVE A FORM.  If you do not see the form you need listed on
the index, it is not yet available via FAX and you should contact
the forms distribution center at 1-800-418-FORMS to order the
hardcopy form.
