ERRATUM

Released: December 4, 2012

By the Chief, Wireline Competition Bureau:
On November 16, 2012, the Commission released a Fifth Order on Reconsideration, FCC 12-
137, in the above-captioned proceedings. This Erratum amends the Fifth Order on Reconsideration as
indicated below:
1.
In paragraph 7, in the first, second and fourth sentence, replace “Form 479” with
“Operating Report for Telecommunications Borrowers”; in the third sentence, delete “Form 479”; and in
the fifth sentence, replace “Form” with “Report”.
2.
In footnote 12, replace “Form 479” with “Operating Report for Telecommunications
Borrowers”.
3.
In footnote 13, the first sentence is corrected to read as follows:
“Marlett notes, “One benefit of requiring a form in the RUS . . . format is that all reports
to the FCC would then contain data in a consistent form that can be readily used and
accessed by the Commission because the carriers that are currently required to file RUS
[Operating Report for Telecommunications Borrowers] will also be providing that form
to the FCC.”
