ERRATUM

WIRELINE COMPETITION BUREAU ANNOUNCES IMPLEMENTATION

OF REVISED INSTRUCTIONS FOR FORMS 479 AND 486 

CC Docket No. 02-6

On November 25, 2011, the Wireline Competition Bureau released a Public Notice, DA 11-1943, 
in the above-captioned proceeding.  This Erratum corrects the Public Notice by including the attached 
instructions to FCC Forms 479 and 486 that were erroneously omitted from the original released document.
