This form is for use by the Broadcast Community to report information or complaints about the operation of unlicensed broadcast stations. Provide detailed information including the name of the operator, if known, how the station identifies itself, station contact information (e.g., address, phone number) if available, time of operation, the Address/location of the station, the operating frequency, and whether the station's operation is causing interference. As a minimum include the operating frequency, city, state and the date and time last heard on the air. The information provided will be automatically forwarded to the closest Field Enforcement Office. Your email address on the second line will enable us to provide you with confirmation of your submission and additional information.
