RESELLER CERTIFICATION LANGUAGE FOR FCC FORM 499-A INSTRUCTIONS

The Wireline Competition Bureau (Bureau) seeks comment on a proposal filed by a group of 8
industry participants regarding revisions to sample reseller certification language and accompanying
sections of the FCC Form 499-A instructions.1 The FCC Form 499-A is used by contributors to the
Universal Service Fund to report their revenues to the administrator of the Fund. In the 2012 Wholesaler-
Reseller Clarification Order
, the Commission directed the Bureau to revise the sample language in the
Form 499-A to reflect the clarifications provided in the Wholesaler-Reseller Clarification Order
allowed contributors to rely on existing sample language until December 31, 2013.2 The Bureau seeks
comment on the proposed revisions submitted by the industry participants,
including whether they should
be included in the 2014 FCC Form 499-A instructions.
Pursuant to sections 1.415 and 1.419 of the Commission's rules,3 interested parties may file
comments on or before September 6, 2013 and reply comments on or before September 13, 2013

Parties who choose to file by paper must file an original and one copy of each
filing. Filings can be sent by hand or messenger delivery, by commercial overnight courier, or by

proceeding in accordance with the Commission's ex parte
