Note: the report descriptions and instructions obtained through the links below are applicable to reporting year 2007 data, which is required to be filed not later than April 1, 2008.
Annual Summary Report: This report, formerly named the "Quarterly Report," collects highly aggregated financial data reflecting the results of accounting, rate base, and cost allocation requirements prescribed in Parts 32, 64, 65, 36 and 69, of the FCC Rules. Prior to 1996, this report was a cumulative quarterly report. Beginning with 1996 data, the 43-01 is filed on an annual basis, showing year-end results. The 43-01 is filed on a study area (jurisdiction) basis.
USOA Report: This report collects the operating results of the carrier's total activities for every account in the USOA, as specified in Part 32 of the FCC Rules. There are three series of tables included within the ARMIS 43-02 report which are mechanized versions of data that were formerly filed as part of the Form M reports. The 43-02 is filed on an operating company basis.
Joint Cost Report: This report contains the breakdown of the reporting carrier's costs between regulated and nonregulated activities as defined in their Cost Allocation Manuals and Part 64 of the FCC Rules. There is a single table in the 43-03, Regulated/Unregulated Data (Table I), which contains regulated and nonregulated financial data. The 43-03 is filed on a study area (jurisdiction) basis.
Access Report: This report shows the separation of the reporting carrier's regulated revenues and costs between the state and interstate jurisdictions, as defined in Part 36 of the FCC Rules, and the distribution of interstate amounts among the access charge categories, as defined in Part 69 of the FCC Rules. There is a single table in the 43-04: Separations and Access (Table I). The 43-04 is filed on a study area (jurisdiction) basis.
Service Quality Report: This report was a quarterly service quality report through 1995. Prior to 1996, installation and repair interval data represented service provided during the current reporting period . Access lines data represented lines in service at the end of the previous year. Trunk blockage data was reported on a monthly basis for the current reporting period. Switch counts were updated quarterly, with the fourth quarter representing year-end counts. Service quality complaints data represented the number of complaints received during the current reporting period. Beginning in 1996, the report is filed annually. The 43-05 is filed at the study area (jurisdiction) and the holding company levels.

- Table I Installation and Repair Intervals (Interexchange Access) contains the installation and repair intervals achieved by the reporting carriers for services provided to interexchange carriers. The data is segregated between switched access and special access services. Row Instructions Column Descriptions
- Table II Installation and Repair Intervals (Local Service) covers the installation and repair intervals achieved by the reporting carriers for local services they provide to both business and residential customers. Row Instructions Column Descriptions
- Table III Common Trunk Blockage reports blockages on common trunk groups between the local exchange carrier's end office and the access tandem. Row Instructions Column Descriptions
- Table IV Total Switch Downtime summarizes the loss of local switch call processing capability, including identification of total downtime durations of less than two minutes. Row Instructions Column Descriptions
- Table IV.A Occurrences of Two Minutes or More Downtime provides details of all occurrences of local switch outages of two or more minutes duration. Row Instructions Column Descriptions
- Table V Service Quality Complaints is a count of the formal complaints raised by residential and business customers in the state and interstate jurisdictions. Row Instructions Column Descriptions
Customer Satisfaction Report: This report was a semiannual service quality report through 1995. Beginning in 1996, the report is filed annually. The 43-06 is filed at the study area (jurisdiction) and the holding company levels.

- Table I Summary Customer Satisfaction Survey contains the results of customer satisfaction surveys. Through 1993, Table I was only filed as a paper report, i.e., it is not available electronically. Also, through 1993, Table I reported customer satisfaction. Beginning in 1994, Table I reports the percentage of customers that are dissatisfied with various aspects of the reporting carrier's service. Row Instructions Column Descriptions
Infrastructure Report: This report provides data regarding the infrastructure of the reporting carrier. The 43-07 is filed at the study area (jurisdiction) and the holding company levels.

- Table I Switching Equipment provides quantities of local switches according to type, e.g., electromechanical or digital stored program control, and by capability, e.g., equal access and ISDN. - Table II Transmission Facilities contains information on interoffice facilities and loop plant, with categories for copper, fiber, analog and digital carrier, and radio technologies. Row Instructions Column Descriptions
Operating Data Report: This report contains statistical schedules that were formerly reported in Form M. All of the tables in the 43-08 are organized by state jurisdiction, and each report only covers the reporting carrier's totals for that state. In order to obtain state totals it is necessary to sum the data for each reporting carrier operating in that state. The 43-08 is filed on an operating company basis.
Forecast Report: This report displays forecasts of expected regulated and nonregulated investment usage at the study area level, for the current calendar year and the subsequent two calendar years. The 495A has been filed since 1989, and provides the forecast and resulting investment allocations incorporated in the carrier's cost support for its access tariffs. The data filed is proprietary, with the exception of some limited data filed in an unrestricted version of the report. The 495A is filed on a study area (jurisdiction), a consolidated access tariff area, and an operating company basis.

- Table I Forecast Usage Schedule for Central Office Equipment and Cable and Wire Facilities by Cost Category contains forecasts of usage for three calendar years following the filing date of the report, for cost categories identified in the carrier's approved Part 32 Cost Allocation Manual that require allocation on the basis of a forward looking allocator. Actual Usage Report: This report is a companion to Report 495A and displays the actual usage of regulated and nonregulated investment at the study area level for the prior calendar year. This report has been filed since 1989, and provides the actual investment and usage data for those cost categories that are allocated on the basis of a forward-looking investment allocator. The data filed is proprietary, with the exception of some limited data filed in an unrestricted version of the report. The 495B is filed on a study area (jurisdiction), a consolidated access tariff area, and an operating company basis.
