COMMENT PERIOD EXTENDED FOR REVISIONS TO FCC FORM 471 INSTRUCTIONS

Comment Date: January 2, 2014The Wireline Competition Bureau (Bureau), after consultation with the Office of Management
and Budget (OMB), is providing this opportunity for interested parties to comment on the information
collection burdens associated with revisions to the approved Federal Communications Commission (FCC)
Form 471 Instructions. On December 18, 2013, OMB approved the revised FCC Forms 470 and 471, and
their accompanying instructions. 1 The Bureau, however, is providing another 14 days for comments on
the FCC Form 471 Instructions because, due to a clerical error, the revised instructions were not posted
on OMB’s website during its 30-day comment period. We note that the Bureau in July sought comment
on the revised FCC Form 471 Instructions, along with FCC Forms 470 and 471, and the FCC Form 470
Instructions.2 Copies of both the approved FCC Form 471 Instructions and the FCC Form 471 are

Filing System (ECFS), or by filing paper copies. See Electronic Filing of Documents in Rulemaking

Electronic Filers: Comments may be filed electronically using the Internet by accessing the
Paper Filers: Parties who choose to file by paper must file an original and one copy of each
filing. Filings can be sent by hand or messenger delivery, by commercial overnight courier, or by
first-class or overnight U.S. Postal Service mail. All filings must be addressed to the
Commission’s Secretary, Office of the Secretary, Federal Communications Commission.

comment are largely the same as those put out for comment in July, except for a few modifications, most of which
are responsive to comments submitted in response to the Bureau’s July Public Notice.
o All hand-delivered or messenger-delivered paper filings for the Commission’s Secretary
must be delivered to FCC Headquarters at 445 12th St., SW, Room TW-A325,
Washington, DC 20554. The filing hours are 8:00 a.m. to 7:00 p.m. All hand deliveries
must be held together with rubber bands or fasteners. Any envelopes must be disposed of
before entering the building.
o Commercial overnight mail (other than U.S. Postal Service Express Mail and Priority
Mail) must be sent to 9300 East Hampton Drive, Capitol Heights, MD 20743.
o U.S. Postal Service first-class, Express, and Priority mail must be addressed to 445 12th
Street, SW, Washington DC 20554.
